^ OK, the name of the theme is Mosaic Mad Blue by IND90.

Actually the one you're seeing above is v2.02, while there was also an updated v2.03. There were some differences between the two & I personally prefer v2.02. So, I uploaded v2.02 HERE. While if you want the updated version, you can get it from IND90's official website HERE
